This manual is completely worthless. Do NOT buy if you are just re-certifying; I really don't know what I was thinking. I gaurantee that you will not learn anything new.

I suppose it is worthwhile for those learning CPR for the first time (at least it's cheap).

This is the textbook that all EMT-B classes use, it's really the only choice you have. I found the companion study guide very helpful for studying. The 9th edition includes a CD and the on-line resources were great too. I especially like the chapter on Terrorist response, rather interesting comment about society and the times we live in.

*Emergency Medicine, A Comprehensive Study Guide* has long been a well-regarded standard emergency text, useful in day-to-day practice as well as for board preparation. This new edition has advanced the enlightened 'evidence-based recommendations' for changes in practice patterns, however, the lack of editorial control has resulted in a book packed with conflicting information. Each chapter is written by a different consultant, and unfortunately not all of them are up on this concept... we therefore get a series of conflicting statements when there is overlap in topics, and there is no attempt to reconcile this variability. Admittedly, opinions in much of medicine do vary; but outright conflict within a few pages in a standard text without discussion does harm to credibility.

Further loss of editorial control is also evident on a close read. It is apparent that a spell-checker was used for proof-reading (substitutions of the nature of "week" for "weak" abound) and in some areas clinical formulae have been mis-stated -- example: calculation of effective osmolarity, pg. 1341 (though correct elsewhere in the book). The index seems somewhat disorganized, with references to isolated appearance of a word (example: see *first* reference listing to "hyperglycemia"... certainly not the definitive source in the text). This problem is not isolated.

The book has expanded by approximately 500 pages over the 4th edition, totalling more than 2000 pages. Its size makes it unwieldly to read or carry; it should have been separated into 2 volumes. Even with that expansion, much useful information is missing. Though descriptions of EKG abnormalities in advanced electrolyte disturbance are adequately described, it certainly would have been an informational asset to actually include a picture of such characteristic tracings. Dermatology is similarly poorly pictured -- there are only 8 pages of color illustrations in the text, with nearly 2 pages wasted on 1)technique of foreign body removal (not needed in color) 2)a color demonstration of SPECT data -- well outside the range of information needed for the E.M. specialist, and not definitive, besides. Even worse, the illustrations interspersed in the dermatology section are simply black and white copies of the color text -- providing only suboptimal repetition.

Bottom line: the *Comprehensive Study Guide* has been, and remains, a valuable text for an overview of the practice of emergency medicine; but either keep your 4th edition, or wait for the 6th.

Living out in the wild, in the wilderness, is a contest with Nature. Most of the time, man wins the contest, but sometimes ... stuff happens.

Chapter One is about Assessment and starts with that key question: scene safe? Then Dr Forgey takes his reader through the ABCD's, vital signs, levels of consciousness, head to toe examination, shock, respiration rates, heart rates, and CPR. (The numbers for chest compressions and breaths has been changed by the AHA since Dr Forgey updated this book, but that is a minor issue.)

Chapter Two is about body system management. The focus of this chapter is on the systems in the head but the abdomen and reproductive system are given sections as well. There is also a very good, short section on poisoning from food poisoning to shellfish poisoning.

Chapter Three covers soft tissue wounds and treatments ... and suturing and stapling.

Chapter Four covers orthopedic injuries from head to foot.

Chapter Five covers bites and stings and anaphylactic shock. Interest-ingly Dr Forgey finds that rubber suction cups are as worthless as mouth suction. His lone endorsement is the Sawyer Extractor (which is available from Amazon.com).

Chapter Six is on infectious disease. Dr Forgey lists the most signif-icant *wilderness* diseases for North America and the world should one be contesting Nature abroad.

Chapter Seven's environmental injuries include hypothermia, heat stress, high altitude related illnesses, and ... being struck by lightning. Step current is caused when lightning hass struck and the current spreads out like a wave across the ground and the victim's feet are different distances from the strike point. Since the body has less resistance than the ground, a circuit is completed.

There are two useful appendices at the end of the book.

I am EMS certified and as a BLS instructor. I had a few quibbles with Dr Forgey such as his choice of prescription medications to list in one of the appendices. However I had no major disagreements and found the book to be more easily readable than any EMS book I have read. Lots of nuts and bolts and no fluff.

Also as I write this review, I am preparing a first aid segment for a TCLEOSE course on mantracking. Dr Forgey's book provided me with a lot of detail and anecdotes to include. However just as the title says this book is about wilderness medicine *beyond* first aid.

I really didn't like this book, and I'm not sure if it is just the topic that I struggled with or if it was the text. I had a tough time reading the book...I didn't like the blank lines to fill in the answers, it seemed distracting to me trying to learn the information and having to stop and fill in the blank as I go along.
As far as the information, it was really very compelete and there were A LOT of practice strips in the text. That part I did find useful. It does take a lot of practice in class, in the field and in clinical settings to learn the cardiolody and ekg intrepretaion. If you can get passed the fill in the blanks part, or if that doesn't bother you than it seems to be a very good book. It just didn't seem to suit my learning style.

This is an excellent review for the NREMT exam for EMT-B certification. It is well organized, helps you pinpoint areas where you need to study more, and is written very much like the actual test.

The questions in these practice tests are much more difficult than those found in online practice tests, which tend to be over-simplified. Just as important, this book is from an authoritative source; I found some answers in the online sources which were just plain wrong.

The NREMT exam is as much about test-taking ability as it is about subject matter knowledge, and that's where this book really helps. In particular, there are many questions on the actual exam where the choices contain information which looks very simple at first glance, but which in fact contains very subtle differences. The questions in this book give you those types of choices, and train you to read each question carefully looking for those nuances which are the difference between a right answer and a wrong answer.

Your first time through the questions in this book may be a wake-up call if you don't really know your stuff. But it's better to get a wake-up call before the test, rather than during the test. I passed on the first try. Good luck!

Arranged by chief complaint, with instant access to the most pertinent information on workup and treatment.

There really is no comparable book in existence. I saw a recent review comparing it to 'Pocket Medicine'? There's no comparison. If you have a patient with chest pain, this book puts all the possible diagnoses in one section with the labs/imaging/physical exam tips that you need to decide which one is most likely...followed by the treatment. With 'Pocket Medicine' you'd have to jump from ACS, to PE, to esophageal spasm, to AAA, to tamponade, and on and on. You better not forget any possible causes because it's not arranged by complaint but by body system (like all internal medicine guides)

I had to write a review because I don't want medical students, interns, or residents thinking the 'Pocket Medicine' will help you in the ER.

'Pocket Medicine' is for internists or your internal medicine rotations.

'Pocket Emergency Medicine' is the ONLY handbook to carry in the ER.
