Editorial Review:

Nursing Care Plans, 6th Edition is the most comprehensive nursing care planning book on the market, with 183 care plans covering the most common nursing diagnoses and clinical problems in medical-surgical nursing. It reflects the most current clinical practice and professional standards in nursing and presents the content in an easy-to-use, reader-friendly format. The book functions as two books in one, with one section including a collection of nursing diagnosis care plans and the other providing a library of disease/disorder care plans that serve as models of care planning for the most common medical diagnoses, medical procedures, and psychosocial conditions seen in nursing practice.

  • Includes the most extensive array of care plans found in any nursing care planning book, with 62 nursing diagnosis care plans and 121 disease/disorder/procedure care plans, for a total of 183.
  • The nursing diagnosis care plan format includes a NANDA definition, a brief explanation of the diagnosis, related NOC outcomes and NIC interventions, related factors, defining characteristics, expected outcomes, ongoing assessment, and therapeutic interventions.
  • The disorders care plan format includes synonyms for the disorder (for ease in cross referencing), a clear and succinct definition of the disorder, related factors, defining characteristics, expected outcomes, NIC interventions and NOC outcomes, ongoing assessment, and therapeutic interventions for each relevant nursing diagnosis.
  • Eye-catching icons distinguish collaborative from independent interventions.
  • Covers adult lifespan issues and issues related to various healthcare settings to equip nurses for a broad range of practice settings, from hospital to home.
  • Provides an alphabetical list of NANDA diagnoses inside the front cover.

  • Includes new disease/disorder care plans for SARS, Lyme disease, West Nile virus, and obstructive sleep apnea.
  • Includes new nursing diagnosis care plans for risk for falls, latex allergy response, nausea, and impaired memory.
  • A revised care plan format makes this user-friendly reference even easier to use.
  • Updated and expanded rationales throughout reflect the latest clinical evidence and clinical practice guidelines.
  • Incorporates the 2005-2006 NANDA nursing diagnoses throughout.
  • NIC and NOC classifications reflect the latest editions of Nursing Interventions Classification and Nursing Outcomes Classification.
  • Sample care plan clips in Chapter 1 help orient the user to the rest of the book.
  • The expanded index includes entries for all nursing diagnoses, medical diagnoses, and synonyms for the medical diagnoses.
  • An expanded Evolve website now features 18 Bonus Care Plans.
  • The revised Online Care Plan Constructor includes the four new nursing diagnosis care plans and offers enhanced functionality, allowing users to save created care plans and export them to their word processing program.

Editorial Review:

Nurses share what it’s like to work with doctors—when it’s like a smooth-running machine, when communication breaks down, and how it directly affects the patient. Hear from people new to the field as well as those who have been in nursing for decades about power differences, gender dynamics, and the challenge of public perceptions.

Edited and introduced by a registered nurse, Reflections on Doctors is a resource for nurses and anyone who wants to better understand the healthcare professionals who care for them.

Editorial Review:

Unlike any other care plan book on the market, this resource contains selected care plans for pediatric, maternity, and psychiatric nursing in addition to medical-surgical nursing care plans for adult care. This is the only care planning resource that allows you to use one book throughout your nursing curriculum. Color-coded medical-surgical, maternity, pediatric, and psychiatric sections help you quickly find each content area while giving the book a full-color look and feel.

  • Clinical experts update each subject area to ensure the most current, accurate, and clinically relevant content available.
  • Each care plan employs a consistent format of Overview/Pathophysiology, Health Care Setting, Assessment, Diagnostic Tests, Nursing Diagnoses, Desired Outcomes, Interventions with Rationales, and Patient-Family Teaching and Discharge Planning.
  • An open and attractive two-color design facilitates quick and easy retrieval of information.
  • Nursing interventions and rationales are listed in a clear two-column format to make the information more accessible.
  • Related NIC intervention and NOC outcome labels are listed for each nursing diagnosis.
  • The Patient-Family Teaching and Discharge Planning section highlights key patient education topics and list resources for further information.
  • Health Care Setting is specified for each care plan, since different conditions are treated in various settings such as hospital, primary care, long-term care facility, community, and home care.
  • Outcome criteria with specific timelines enable nurses to set realistic goals for nursing outcomes and provide quality, cost-effective care.
  • Detailed rationales for each nursing intervention help you to apply concepts to clinical practice.
  • Includes the most recent NANDA Taxonomy II nursing diagnoses.
  • Separate care plans on Pain and Palliative and End-of-Life Care focus on palliative care for patients with terminal illnesses, as well as relief of acute and chronic pain.

  • A new "Overview/Pathophysiology" heading helps you easily locate this key content.
  • Nursing diagnoses listed in order of importance/physiologic need helps you learn about prioritization.
  • All content has been thoroughly updated to cover the latest clinical developments, including the most recent JNC7 hypertension guidelines, the latest breast cancer screening and treatment information, revised cholesterol parameters, new drug therapies, and much more.
  • Patient teaching content and abbreviations have been thoroughly revised to reflect the latest JCAHO guidelines.
  • Expanded and clarified rationales help you understand each intervention more clearly.

Improving Communications in the Health Care encounter 5 out of 5 stars.
6 of 6 people found this review helpful.

Most patients feel rushed and not heard in their encounters with health care providers. This clearly written "how to" book provides principles and examples of conversation that can improve the quality of communication around life style change between provider and patient. Listening rather than telling, and guiding rather than directing or preaching are difficult skills for health providers. Giving the patient "a voice and a choice" in making changes in their unhealthy behaviors is essential to successful outcomes. This practical guide to health behavior change has made a difference in my practice when patients give me permimssion to discuss their drinking, smoking, drug use, over eating, fitness and seat belt use with them.

Editorial Review:

Much of health care today involves helping patients manage conditions whose outcomes can be greatly influenced by lifestyle or behavior change. Written specifically for health care professionals, this concise book presents powerful tools to enhance communication with patients and guide them in making choices to improve their health, from weight loss, exercise, and smoking cessation, to medication adherence and safer sex practices. Engaging dialogues and vignettes bring to life the core skills of motivational interviewing (MI) and show how to incorporate this brief evidence-based approach into any health care setting. Appendices include MI training resources and publications on specific medical conditions.

The Case Management Bible 5 out of 5 stars.
51 of 53 people found this review helpful.

I am not a case manager, however I work with them on a regular basis. I have found this book extremely helpful in understanding the field, the impact case management can and does have on the health care delivery process, and how it helps improve outcomes and contain costs, especially in critical and chronic care cases. This book is an invaluable tool for case managers and anyone who requires proven, nuts and bolts information on the process of case management and the role of case managers in health care. Lots of basic, hands-on, how-to information.

Excellent or Flawed? - Depends on your perspective 3 out of 5 stars.
12 of 12 people found this review helpful.

It's a bit unusual for me to agree with customer reviews that both praise and criticize a book, but in this case I do. On the one hand, Rollnick et al, have done an amazing job of applying motivational interviewing and stages of change theory to the short interchanges common with patients in a medical setting. For those readers already well-versed in stages of change, this is an excellent and thought-provoking approach. I believe many "counselors" of various stripes would benefit from the applications advocated in this book.

On the other hand, I found the theoretical foundation wholly inadequate. While I appreciated the attempts of the authors to carefully distinguish between evidence-based substantiation of their guidelines and the weaker suggestions based on clinical practice, I felt that there was a preponderance of the latter.

I was also overwhelmed by the repetition included in the three final "application" chapters. Surely there is a better way to present this material! Frankly, the final chapters are so tedious to read that I suspect the average medical professional tends to conclude this volume with a less-than-enthusiastic feeling regarding the guidance.

My advice is to read through chapter 5, at the most, and to consult chapters 6-8 only after encountering specific problems in applying the techniques provided.

Editorial Review:

This book is the first in its field to offer practitioners a patient-centered method for changing a patient's health behavior. Using brief, structured consultations with the client, the practitioner encourages the patient to take charge of decision-making concerning his/her health. The text clearly outlines the fundamental principles behind the method while applying it to practice.
