I am a student studying health services. Helpful Book for Understanding the Long-Term Care System 5 out of 5 stars.
7 of 7 people found this review helpful.

This was assigned as a text book and it was a good one. This text is packed with information on the various ways long-term care is delivered in the US today. After one reads this book they will understand the complexities of the long-term care health care system. Pratt discusses, funding, organizational structures, regulation, accreditation, and how the different types of long-term care fit into the whole system.

Each chapter builds to provide a complete picture of long-term care and understanding how all the parts fit together to make a whole system. This book is strongest is in its discussion of funding structures and how funding impacts service delivery. The strongest chapters were on quality and the various ways that quality is currently measured. Pratt points out that measuring process and systems as is currently and most commonly done does not always guarantee quality outcomes.

Chapter 2 which discusses the "ideal" long-term care health system and Appendix A: Criteria for Designing or Evaluating a Long-Term Care System are worth the price of this book because the information contained in these pages can assist long-term care administrators strengthen their facilities and service delivery and policy makers in understanding the complexities of long-term care systems.

Editorial Review:

This book provides a solid foundation on which to build expertise, understand the differences and similarities among the many long-term services, how the various segments of long-term care fit together to form an overall system, and the skills needed to succeed in that system.

Editorial Review:

As recent advances in medicine contribute to people living longer and living with chronic illness in the home, the role of the family caregiver has become more common. Furthermore, due to varying family structures and living situations it is not always the parent or adult child who takes on the caregiver role. The traditional role of the adult caregiver increasingly falls on the shoulders of children, adolescents, and emerging adults. Also, in cases where parents are unable to take care of their children, grandparents take on the role of adult caregiver for their grandchildren. A caregiving role at any point in life may affect a person's development and add stressors, but also may create rewards that influence the caregiver's identity and well-being."How Caregiving Affects Development: Psychological Implications for Child, Adolescent, and Adult Caregivers" examines these effects using a lifespan development framework. Each chapter presents theory and empirical research on caregiving from a different phase in the lifespan including childhood, adolescence, emerging adulthood, and young, middle, and older adulthood. Within the context of the caregiver's life the chapter authors examine how the role of caregiver affects their development. The topics include the effects of early caregiving on well-being, school performance, filial responsibility, felt obligation, coping with stresses of multiple roles in middle adulthood, and intergenerational reciprocity. Challenges and rewards specific to each stage in life are examined, and some chapters also provide a comparison with the experiences of noncaregiving peers. This book offers a new and exciting look at often overlooked family caregivers and introduces a set of challenging questions that will guide future research in the field.

A Great Start 5 out of 5 stars.

Had submitted an article of my own for this anthology and had to withdraw it for reasons which had nothing to do with this publication. This is an EXCELLENT publication and I am so very happy for the publishers that they decided to go for such a worthwhile effort. Wonderful stories, etc - great source of information from the mouths of those most closely informed by this disease. I highly recommend this book, and the coming series, to anyone having a family member with one of these diseases, or caring in general about others and wanting to understand as much as possible. Shelby Staples

Editorial Review:

The incidence of Parkinson's is growing as the number of older adults making up the general population increases. The demand for nurses who specialize in the care of patients with movement disorders is also increasing and it is crucial that they have a comprehensive manual for patient care.

Lisette Bunting-Perry and Gwyn Vernon have years of experience working with Parkinson's patients and have put together a team of internationally renowned clinicians who provide a truly comprehensive review of the information you need for the care of patients with this complex clinical profile. Comprehensive Nursing Care for Parkinson's Disease covers what is presently known about the disease and current evidence-based standards of care, including management through deep brain stimulation, palliative care for frail older adults with Parkinson's, medication management, nonmotor complications and psychosocial issues, and complementary and alternative approaches.

This is an indispensable bedside companion in caring for those with Parkinson's as well as a much needed catalyst for the evaluation of future patient needs and the further development of comprehensive models of care.

Intergenerational interdependence 5 out of 5 stars.
3 of 3 people found this review helpful.

Reviewed by Richard R. Blake for Reader Views (10/07)

Drs. Miriam K. Aronoson and Marcella Bakur Weiner have written "Aging Parents, Aging Children" for the average person faced with a care-giving challenge. The book is filled with practical examples and dramatic scenarios to help the reader identify with others who have, or are experiencing, similar problems or challenges.

Early chapters include helpful information pointing out diversity among families, differences in philosophy, beliefs, personality traits, and family dynamics. These all play a part in the approach chosen to provide the best possible care.

As changes in physical health, emotional stability, or mental faculties, take their toll, it is important to begin an assessment process. This will help the family or caregiver size up the current condition through diagnosis, prognosis, treatment and aftercare needs. Excellent charts and tables provide an overview and checklist for this assessment and setting up a plan of care. Once this has been done immediate concerns, logistics, legal issues, and finances all come into play.

I especially appreciated the chapter dedicated to discussing dementia and Alzheimer's disease and the suggestions for placement versus home care. Symptoms, causes, and treatments are all considered in this informational and helpful presentation.

A concern often overlooked is the care and welfare of the caregiver. This chapter is of particular importance to everyone reading the book. Self-care is an important part of the process of renewal, nurture, and balance. The final chapter titled "Demystifying the Maze" is a helpful glossary of important elder care language.

Internationally acclaimed as experts in the area of elder care, Aronson and Weiner, collaborate to bring the reader insight, confidence, compassion and hope in caring for their loved one. The authors assist the reader faced with navigating the maze of systems and services, the challenge of relationships, resource limitations, and expectations of family and the importance of self help.

Thorough in presentation, written in an easy-to-understand style, "Aging Parents, Aging Children" is a practical prescription for an escalating pandemic of eldercare challenges.

Editorial Review:

Sandwiched between the escalating needs of their aging relatives and their own children, today's adults are caught in an intergenerational squeeze. This upbeat self-help book features case examples that speak directly to Boomers and other caregivers, and addresses the feelings at play within themselves and their family system. It offers practical advice and methods to help families cope better during this potentially stressful period in life.

Editorial Review:

Like its popular predecessor, the new edition of Bathing Without a Battle presents an individualized, problem-solving approach to bathing and personal care of individuals with dementia. On the basis of extensive original research and clinical experience, the editors have developed strategies and techniques that work in both institution and home settings. Their approach is also appropriate for caregiving activities other than bathing, such as morning and evening care, and for frail elders not suffering from dementia.

For this second edition, the authors have included historical material on bathing and substantially updated the section on special concerns, including:

  • Pain
  • Skin care
  • Determining the appropriate level of assistance
  • Transfers
  • The environment

An enhanced final section addresses ways to support caregivers by increasing their understanding of the care recipient's needs and their knowledge of interventions to improve care and comfort. It also emphasizes self-care and system-level changes to promote person-directed care. Several chapters include specific insights and wisdom from direct caregivers.

Editorial Review:

"At present nursing homes are designed . . . like outmoded zoos. Residents are kept in small rooms, emotionally isolated. Occasionally they are visited by family members who reach through the bars and offer them treats. Aides keep their bodies clean and presentable. . . . America invests huge amounts of money to maintain the body while leaving the person to languish, cut off from all they love."—From Nobody’s Home

After caring for his mother at the end of her life, Thomas Edward Gass felt drawn to serve the elderly. He took a job as a nursing home aide but was not prepared for the reality that he found at his new place of employment, a for-profit long-term-care facility. In a book that is by turns chilling and graphic, poignant and funny, Gass describes America’s system of warehousing its oldest citizens.

Gass brings the reader into the sterile home with its flat metal roof and concrete block walls. Like an industrial park complex, it is clean, efficient, and functional. He is blunt about the institution’s goal: keep those faint hearts pumping and the life savings and Medicaid dollars rolling in. With 130 beds in the facility, the owner grosses about three million dollars annually. As a relatively well-paid aide, Gass made $6.90 an hour.

Seventeen of the twenty-six residents on Gass’s hall were incontinent, and much of his initiation to the work was learning to care for them in the most intimate ways. One of the many challenges was the limited time that he had available for each of his charges—17.3 minutes per day by his calculation. Even as he learned to ignore all but the most pressing demands of the residents, he discovered the remarkable lengths to which aides and their patients will go to relieve the constant ache of loneliness at the nursing home.

With Americans living longer than ever before, elder care is among the fastest growing occupations. This book makes clear that there is a systemic conflict between profit and extent of care. Instead of controlling costs and maximizing profits, what if long-term care focused on our basic need to lead meaningful and connected lives until our deaths? What if staff members dropped the feigned hope of forestalling the inevitable and concentrated on making their charges comfortable and respected? These and other questions raised by this powerful book will cause Americans to rethink how nursing homes are run, staffed, and financed—as well as the circumstances under which we hope to meet our end.

Two of my grandparents had Alzheimer's, but I never understood how devastating the disease can be until I read this book. If you're a caregiver, you'll find inspiration as well as practical advice on coping with the disease. If you know anyone who suffers from Alzheimer's, you'll gain a better understanding of the disease and learn how you can help the patient and caregiver. This book would make a thoughtful gift for anyone who is dealing with Alzheimer's.

Informative and Helpful 5 out of 5 stars.

This is a moving account of what families go through with this illness. It shows how important a support system is and the best way to utilize that support.

Living the author's journey 5 out of 5 stars.

My wife and I had the privilege of knowing and loving Gordon and Jean Pitzer during this crisis period when Gordon was ill. When Jean decided to journal her account to make sense of it all, we, along with others, encouraged her to put it into book form. Then, she and my wife, word by word, edited it and a publisher picked it up. It is an explicit account of what both an Alzheimer patient lives and what his caregiver endures. Anyone in this situation will find value in Jean's poignant, reflective experience and come away with a sense of hope.
