Nursing test study guides must provide correct information not just to help the reader pass the test, but to aid the reader in improving nursing care.

This book's Q&A format will not aid the nurse in improving nursing care. For example on page 40 the the answer to the question "What is the suspected diagnosis if a patient's blood pressure drops significantly with administration of nitroglycerin in the setting of an acute MI?" is "Inferior wall MI with right ventricular involvement." A better answer would include the recommendation of volume replacement and why instead of addressing the treatment under the cardiogenic shock question.

I checked the copyright date twice to confirm this was published in 2006 before I decided to write a negative review.

The book uses AHA 2000 guidelines instead of the 2005 guidelines which were widely available by August 2005. For examples: The book recommends the non-current practice of ventilation to compression ratio of 1 to 5 instead of the current 2 to 30; the old compression rate of 80-100 instead of the current compression rate of 100; and stacking of 3 shocks instead of 1 shock then start CPR.

What is it with the authors love of Lidocaine with all those questions about it? Lidocaine is NOT a first line drug choice for anything cardiac, thus it is NOT the treatment of choice for hemodynamically stable ventricular tachycardia (page 48). Nor is the drug "likely to be ordered" after " a patient is brought back to a normal sinus rhythm following defibrillation" (page 63).

In the answer about the preferred agent of choice in treatment of atrial fibrillation ( page 51), procainamide has not been recommended 2nd line treatment for 6 years or more.

Amrinone has not been that drug's name since 2000 - try using Inamrinone as the correct drug name. (Name was changed because of the confusion with amiodarone.)

What is with the questions about using LDH in diagnosing AMI in these days of bedside Troponin testing?

The Q&A on page 60 about "What is the treatment of Torsades de Pointes?" really misses the point about using magnesium. This poor answer was my breaking point and why I felt this book needs a warning label. Of course any books that list pressure on the eyeball as a common Valsalva maneuver, page 61, is asking for lawyers to attack.

A book that claims to address CCRN test should employ a better editor. For examples, EKG should be ECG, Troponia is the Spanish spelling for Troponin, and using "b-blockers" mixed with beta symbol-blockers mixed with "beta-blockers".

    The ECG in Acute MI provides a quick reference text for use in critical clinical situations in which accurate ECG interpretation will lead to more rapid recognition of appropriate candidates for reperfusion therapy, as well as a text for more detailed study of electrocardiogram interpretation. This manual is intended for all clinicians that may have to recognize and treat an acute myocardial infarction (AMI). Thus, this book is for emergency department physicians, cardiologists, critical care physicians, hospitalists, internists, family practitioners, physician assistants, cardiac care nurses, residents and medical students. The text assumes a basic clinical understanding of electrocardiograms and common terminology, and focuses on outlining the essentials of ECG interpretation and how they may be used to accurately diagnose AMI, as well as to facilitate appropriate and timely reperfusion therapy. In this manual, reperfusion therapy includes the use of thrombolytics and percutaneous coronary intervention (PCI), including angioplasty and stent placement.
    The authors utilize a well-organized format of concisely written text with key points highlighted and numerous clinical and electrocardiographic demonstrations included in every chapter in order to illustrate core concepts, and allow for quick reference. The text is divided into six sections for rapid review and easy access: General Issues of ECG Morphologies and the Diagnosis of AMI; the ECG Morphologies of AMI broken down by anatomic distribution (i.e. anterior MI, Inferior MI, etc.); Conditions that can obscure the diagnosis of AMI, including ECG pseudo-infarction patterns and AMI look-alikes; Other issues including atypical presentations, biomarkers and echocardiography as used in the reperfusion decision; and the management of AMI.
    The ECG in Acute MI includes more than 300 12-lead ECGs, over 200 of which have accompanying case histories to help the reader understand the ECG in a clinical context. Many of the case examples include angiographic findings that serve to cement the reader's understanding of the ECG manifestations of anatomical disease. The chapters and representative example cases are cross-referenced extraordinarily well, facilitating review and comparison in an emergency setting. In addition, each chapter contains an extensive annotated bibliography outlining the pertinent studies previously published, including interpreted methods, findings and comments. The value of including a detailed description of the evidence based literature on which assertions in the text are made, cannot be overstated. Overall, there are more than 500 cited references for the text.
    The ECG in Acute MI is well organized, thorough and an easy to use manual, both for the seasoned physician and the clinician in training. Although a manual focusing only on ECG interpretation seems a bit daunting at over 300 pages, the concise format of the text makes reading or reviewing chapters either to refresh prior knowledge or verify findings of an acute ECG in the clinical setting to confirm an interpretation entirely possible. While the reviewer had been comfortable with her ability to interpret ECG's in the acute setting, she was pleasantly surprised at just how much she learned from this manual. The authors set out to review and highlight the key aspects of ECG interpretation in order to more accurately diagnose acute MI and facilitate appropriate and timely therapy. The authors focus on educating the reader on the more sophisticated interpretation of subtle, non-diagnostic and atypical electrocardiographic findings of infarction, beyond the much more easily recognized "tombstoning" pattern of AMI. Additionally, the case based discussion utilizing serial ECG's emphasizes the dynamic nature of the electrocardiogram in acute myocardial infarction. This has led the reviewer to be much more suspicious of subtle hints on an initial ECG and the value of repeated examination in the acute clinical setting. The drawback of a text focused on subtle and atypical ECG findings is that the manual is occasionally tedious in its' detail and tiresome in its' description of differentiating characteristics of subtleties.
    Although there are several excellent manuals for ECG interpretation available in the medical literature, the ECG in Acute MI is superior in its utilization of clinical correlation and evidenced based theory. The ECG in acute MI would be an excellent addition to any emergency department library, and would easily lend itself to the development of an advanced ECG interpretation module for residency curriculum. The authors have added a unique and tremendously valuable resource to the reference texts available to practicing clinicians.

    Quick and accurate interpretation of an ECG is vital in determining the use of reperfusion therapy. Learn to provide appropriate and timely therapy through morphology pattern recognition of ischemia, infarction and pseudoinfarction. This text is punctuated by subtle ECGs illustrating the anatomy and reciprocity of leads, enabling analysis and interpretation through spatial perspectives and logical efforts rather than rote memorization. Clinical context is provided through sample ECGs coded by type, and brief and pertinent case histories. The book also features an annotated bibliography that summarizes major clinical trials, as well as cross-sectional diagrams of the heart. A Brandon-Hill recommended title.

    This book is an invaluable review of neurologic critical care in the intensive care unit. The emphasis is on management in day-to-day practice. Part I is devoted to basic management of critically ill patients with acute neurologic diseases. Part II covers the technology and procedures used in the intensive care unit. Part III discusses the management of specific disorders in critical care neurology. Part IV deals with the management of postoperative neurosurgical complications. Part V covers the management of systemic complications. The final section, Part VI, discusses decisions at the end of life and other responsibilities. An appendix is devoted to the calculation of important variables and provides reference tables for daily practice. For quick reference in the intensive care unit the most useful tables and figures have been extracted by the author and reprinted in an accompanying pocket-sized booklet.
    For this thoroughly revised second edition, there are five new chapters on the organization of the intensive care unit and code of conduct of the neurointensivist, acute spinal disorders, the management of common postoperative neurosurgical complications (two chapters), ethics and legal problems, and withdrawal of life support. Also new to this edition are algorithms on outcome prediction in the specific disorders.
    This scholarly and yet accessible and practical book spans the entire field of acute neurology but remains cohesive as it is single-authored.     Fits in my pocket and has some info, but nothing this compact can have everything. Not as much on differential diagnosis, but it gives me a place to start. As with most of medicine, you end up reading more in other books anyway.
