    I like to understand why I do things, and this book certainly answered my questions. Every time I take ACLS I memorize the algorhythms, and a week later I start to forget! This book gives background information that explains and cements the guidelines. Recently I started teaching ACLS. Students ask a lot of questions, and that's why I picked up this book. It helped me discuss questions in a much more meaningful way. The only reason I give it four stars is because I thought it could have been a little richer - it is incredibly easy, but mostly that's good!

    This comprehensive text of cardiovascular medicine is useful for both student and doctor alike. It presents clear, succint information about all major elements of cardiovascular medicine - well presented in easy to find sections. It divides the topic along the lines of acute vs chronic and also discusses complaints based on their anatomical location (e.g. diseases of aorta and pericardium). There is also a comprehensive section on procedures and tests. All in all a very useful addition to anyone's library.

    Colpo writes in clear, engaging and flowing prose. The title does not convey the many other health myths he takes to task using Randomized Clinical Trials as much as possible and keeping an eye on statistical significance. Here is a partial list of the most striking conclusions drawn by the author:

    1. Higher than average cholesterol is protective against CVD in those over 50, a population that is much more prone to CVD than young people. The very slight evidence for lower cholesterol being protective appears only in much younger subjects, who are at very low risk in the first place.

    2. High cholesterol is protective against infection, including even by HIV. Low cholesterol, below 160 mg/dl is associated with increased mortality in all age groups but markedly so in the very old. It's worth noting here that most lab tests show reference ranges for total cholesterol that go from 200 mg/dl down to zero, that is, having zero total cholesterol, which would certainly be fatal, falls within the normal range given by most testing labs.

    3. The supposedly heart-healthy polyunsaturated and monounsaturated fats are more prone to oxidation than saturated fats. Oxidized LDL is an emerging risk factor thought to aid in the formation of arterial plaque. The body incorporates dietary fats into LDL particles. Thus, consuming the more readily oxidized fats is likely to increase CVD risk by raising oxidized LDL levels, which by the way are not influenced in the least by the level of LDL cholesterol. Saturated fats, due to their lack of double bonds, are more stable and less prone to oxidation. Polyunsaturated are the most easily oxidized followed by the monounsaturated fats. Skip the vegetable oils says Colpo even the ones free of trans fats.

    4. High fasting blood sugar is a much better predictor of CVD as compared to standard cholesterol tests, which are almost worthless, yet most people have never had their fasting serum glucose measured. It's an inexpensive test that can be performed by any lab.

    5. The French paradox is not so paradoxical. The French have one of the lowest rates of CVD in Europe and yet eat the most saturated fat. According to Colpo, wine consumption is not the correct explaination as the Italians also drink lots of red wine. It only seems paradoxical if one believes the dogma that saturated fat is a killer. Colpo argues that stress is a true risk factor. Stress is lowered and the post-meal rise in blood sugar is muted when one takes a relaxed attitude towards dining, as the French are famous for.

    6. Vegetarianism is not so healthy. Colpo does advocate eating lots of fresh fruits and vegetables but shows that meat is also of great value. Studies showing the benefit of excluding meat are subject to confounders (vegetarians excercise more and smoke less, for example) that when corrected for result in no benefit to vegetarians.

    7. Colpo is a low carb advocate in order to control blood sugar but he is not fond of extreme ketogenic diets.

    8. High iron stores are a risk factor for CVD. In Colpo's view this is the reason pre-menopausal women (due to monthly blood loss) have a lower CVD risk than men and post-menopausal women. Pre-menopausal women retain much less iron, which is a pro-oxidant. See point 3 above. Colpo suggests testing serum ferritin levels to determine if one has too much iron. Colpo thinks the standard upper limit given for ferritin by most labs is way too high. So, if your doctor says you have normal iron stores check the reading yourself. Colpo also suggests methods to lower iron levels if too high.

    How do you know you're reversing heart disease? 3 out of 5 stars.

    How can you really know whether coronary heart disease is truly being reversed? Does just surviving until your next birthday suffice? Does not developing symptoms tell you anything? Does even losing 10 lbs really tell you whether coronary plaque has shrunk?

    The answer to all these questions, of course, is no. In past, it hasn't been an easy prospect to prove that heart disease is reversed. It's therefore an easy thing to claim. As a result, many programs cropped up in the last 30 years of the last century that made extravagant claims to reverse heart disease. I believe that Dr. Whitaker's well-intended book is among this genre.

    The book has its strong points. Dr. Whitaker does a great job of pointing out the terribly misguided approach of conventional invasive cardiology and bypass surgery, its tragic overuse and abuse, and how so-called medical therapy is nothing more than palliative treatment for a relently progressive disease. He also does a good job of discrediting the conventional low-fat diet approach to heart disease prevention, a diet that frequently precedes your heart attack and diabetes.

    I do believe, however, that there's much more of the story yet to be told and Dr. Whitaker's book falls short.

    The Great American Heart Hoax: A Wonderful Book 5 out of 5 stars.

    It's a rare book that can be both horrifying and empowering at the same time. "The Great American Heart Hoax," by renowned cardiologist Dr. Michael Ozner, is one of those books.

    "Heart Hoax" is horrifying because it reveals many troubling facts about America's cardiac surgery industry. As a longtime medical advocate and critic of our healthcare system, I've known that too many heart surgeries are being performed in America; I just didn't know how many too many! In his book, Dr. Ozner reveals that, of the more than 1.5 million angioplasties and coronary bypass surgeries performed annually in the U.S., fully 70-90% are not only unnecessary, but are also detrimental -- and in some cases, even cause death. I knew that many diagnostic tests expose patients to radiation, but I didn't know that the 64-slice CT scan -- so often used to detect clogged arteries -- gives a patient the equivalent of the radiation from FIVE HUNDRED X-RAYS. And while I had known that the simple cholesterol test, which doctors routinely use as a predictor of cardiac risk, does NOT give the full picture, I did not know that it correctly predicts risk IN ONLY 40% of cases -- AND that there is another blood test (which doctors normally do NOT use), which can correctly predict cardiac risk in 90% of patients. All of these facts -- and more -- add up to what Dr. Ozner refers to as the "Great American Heart Hoax." The truth, writes Dr. Ozner, is that invasive cardiologists HAVE KNOWN many of these "dirty secrets" for years, but have chosen to ignore them, in the unholy goal of "following the money."

    So, why do I say that this book is also empowering? Because, the exposé takes up only one part of the book. In the second half, Dr. Ozner gives us the "good news," by laying out his solution: a 10-step prevention program he has used successfully with his patients for nearly 30 years. It combines the Mediterranean Diet -- with lots of fruits, vegetables, whole grains, fish and healthy fats -- with nutritional supplements, exercise, stress reduction, and lots more. After reading this book, you will know how to prevent a heart attack and, if you have already had one, how to greatly reduce your chances of ever having another.

    This is a very important book. I cannot recommend it more highly. I have been recommending it to every one of my friends and relatives who are at risk, as well as those who are already (perhaps wrongly) taking statin drugs preventatively. "The Great American Heart Hoax" shows us all a better way.

    I bought this book after i bought the MKSAP. I think its much better than MKSAP. It is very readable and gives you a good summary of what you need to know without going through all the painful studies that you would normally read through in a standard text book.For residents I think its the best book.I will update this review as and when I start answering the board questions.

    A cardiologist tells how to keep yourself healthy. 5 out of 5 stars.

    Cardiologist, Dr.Stephen Sinatra, is best know for his excellent book The Coenzyme Q10 Phenomenon which discusses in detail the role of Coenzyme Q10 (CoQ10) in maintaining a strong, healthy heart. While CoQ10 is indeed essential for heart health so is the less well known nutrient L-carnitine. L-carnitine is so important to our bodies -- especially our hearts -- that our bodies manufacture it every day. Unfortunately, however, many people don't manufacture enough L-carnitine to maintain optimum health and some people manufacture so little L-carnitine that their heart muscles become severly weakened for lack of it. Fortunately, L-carnitine is readily available as a nutritional supplement so that anyone who is deficient in this important nutrient can remedy this deficiency by taking supplemental L-carnitine. For people who are deficient in L-carnitine, supplementation can significantly improve their health, and for those who are severly deficienct it can even save their lives. All of this is discussed by Dr. Sinatra in highly-readable terms in this excellent book. Incidentally, L-carnitine not only helps keep our hearts healthy and gives us energy it also helps us burn fat and thus can contribute to weight loss. All health conscious people would do well to read this important book.
