The only care plan book you'll need! 5 out of 5 stars.
10 of 10 people found this review helpful.

After receiving a recent email asking me which care plan book I'd recommend to a nursing student who was staying up very late working on her care plans, I told her I'd write a review of the book that was my best friend on the night before clinicals. This is it.

I had two care plan books that were not working for me so I spent about 8 hours one weekend in Barnes and Nobles looking through every single book that had on the subject. When I got to this one, I didn't need to look any further. The diagnoses are in alphabetical order, with:
~the definition of the condition,
~defining characteristics both subjective and objective (to jar your memory of your interview of the patient pre-clinical),
~and a big one, Related To Factors (r/t) [which can be a struggle fine tuning as a nursing student];
~Client Outcomes [Client Will (Specify Time Frame)--answer is--"Ct. will____________(something that is obtainable and measurable) during my shift",
~Nursing Interventions and Rationales (in italics.) On my Client Care Plans forms, rationales were called "Scientific Principle (Documentaton Source)" and my instructor wanted the page number as well.

***Please note: I've noticed that there is an 8th Edition available now for purchase. I don't see how they could improve on this text; however, I would probably recommend the newest edition. If you already have the 7th Edition, I would say you probably don't need to buy the 8th edition. It's your call.

Good luck future nurses and Soar!

Editorial Review:

NURSING DIAGNOSIS HANDBOOK, fifth edition, is a convenient reference to help nursing students and practicing nurses select a nursing diagnosis and write plans of care with ease and confidence. The book provides care plans for every NANDA diagnosis and provides a quick access index of appropriate nursing diagnoses for over 1,200 clinical entities including signs and symptoms, medical diagnoses, and surgeries across the life span. The revision of this successful book will incorporate all of the new and revised NANDA diagnoses from the 2000 meeting, as well as further integration of both the NIC and NOC taxonomies. The new edition will also indicate evidence-based interventions and rationales with a margin icon, and a two-color design will readily identify important content and increase ease of use. A new MERLIN website will provide students with the opportunity to create customized plans of care for their clients.

Takes the Intimidation Away for Medical Beginners 5 out of 5 stars.
46 of 47 people found this review helpful.

Very few books actually take the time to hold your hand without simultaenously patronizing you. One might see this text as a doorway to the entrance of medical care. I am a newly trained Navy Hospital Corpsman (military medic), and right out of training, I am already expected to be familiar with patient Sickcall procedures and writing SOAP notes. This book greatly helped me in taking the patient's basic history/exam and provides common assessments to choose from. It also reinforces it's contents with small quizzes after every chapter. Numerous pictures and examining techniques are also clear and concise. Keep in mind, however, that it's "user-friendly" demeanor makes everything simple and to the point; I've learned that nothing in the medical field is simple. For more information on disorders, diseases, or illness, I suggest using traditional medical references, like Mosby's. All in all, I commend this book's ability to boost a medical practitioner with little experience up to one of professional competence and skill. And, it's fun!

Editorial Review:

Reviews important details of taking patient histories and doing physical assessments. Includes photographs, numerous halftone illustrations, and summaries of key points. 22 U.S. contributors. Softcover. DNLM: Nursing Assessment--handbooks.

Diagnostic Tests by SPRINGHOUSE 5 out of 5 stars.
7 of 7 people found this review helpful.

This is an excellent reference guide on the rationale and
specific details of diagnostic medical tests both simple and
complex. The authors provide an extensive forward, a discussion
of collection techniques and precautions. For instance, the
bone marrow aspiration and biopsy is a moderately complicated
procedure aimed at diagnosing leukemias, granulomas, pernicious
anemia, primary and metastatic tumors, the staging of Hodgkin's
disease and evaluation of chemotherapy effectiveness. The
procedure is contraindicated in patients with severe bleeding
disorders. Yellow marrow contains fat cells and connective
tissue; whereas, red marrow contains hematopoiec cells, fat cells
and connective tissue. A detailed Histology of the bone marrow
specimen may detect myelofibrosis, granulomas, lymphomas or
cancer. A hemotology blood and bone marrow report is generated
by the blood laboratory. It shows details of the cell type
and applicable levels of myeloblasts, promyelocytes, myelocytes, metamyelocytes, polymorphs, eosinophils, basophils, lymphocytes,
monocytes, plasma cells, pronormoblasts, basophilic normoblasts,
polychrom-Normoblasts, Orthochrom. Normoblasts and Reticulum
cellular structures. A diagnostician's detailed interpretation
usually follows the detailed description of cellular types.
The SPRINGHOUSE work explains the various procedures and
benefits of doing a plethora of diagnostic tests. This work
is invaluable in helping the patient understand the procedure
and its implications, benefits and contraindications. The
information content in this book can help the patient
immeasurably in formulating intelligent questions to pose
to the medical provider on a continuing basis. This book is
worth the price for all of the above reasons. It is a worthy
addition to any personal medical reference library.

This book is a lifesaver. I work in Pre-Admission testing, and need to review lab tests all the time. The book is clear and easy to use. I like the lists of what high and low values can indicate.

I'm a nursing student and I don't think this book was detailed enough. This book did not give the type of information I needed for evaluating labs. There were not interventions listed either. Iw ould suggest finding another book.

As a first year nursing student I need all the help I can get! This book is awesome and great for doing rationals and care plans. I especially like that it gives reasons and dx for why the lab values may be out of the normal range.

A great book for a great price 5 out of 5 stars.

Bad for Care Plan Writing; Good for Understanding Nurs Dx 3 out of 5 stars.
9 of 9 people found this review helpful.

If I need to write a care plan, this is NOT the book to get. I don't care for its interventions, and no rationales are provided. There are better books out there (check out the Sparks & Taylor book which is 5-star rated).

However, this author knows nursing diagnoses inside and out, and the true value of this book are in the Author's Notes where she shares her insights on what each diagnosis really means, and why it is different from similar NANDAs. I picked up a used edition just for the Author's Notes, and her insights regarding non-NANDA diagnoses and other esoteric stuff.

This book also presents Defining Characteristics very clearly, so you know what major characteristic must be present to justify the dx, and what minor characteristics are often present.

This pocket-sized book with spiral binding is easy to pick up and use. I would not purchase the larger, full-blown version--less easy to use, more worthless verbiage, and has essentially the same Author's Notes as the smaller version.

Bottom line: If you need one or two books to help you write care plans for nursing school, don't get either version of this book. There are better ones available. If you want a third book to help you delve deeper into the meanings of nursing diagnoses, go with the pocket-sized version.

Editorial Review:

ChesPenn Health Services, Chester, PA. Brandon/Hill Nursing List first-purchase selection (#216). Pocket-sized quick reference for nurses and students. Covers the steps of nursing diagnoses and contains diagnostic clusters. Previous edition: c1999. Wire-spiral binding.

An Excellent Companion 5 out of 5 stars.
5 of 6 people found this review helpful.

I am a first year nursing student and I must say this book has been an excellent reference guide. No more lugging around huge nursing books for reference purposes. The book is clearly worded and is illustrated to help the beginner. The organization of the book is excellent as well, allowing you to find what you need to know in a hurry without having to flip through hundreds of pages found in bigger books. An excellent guide for anyone in the healthcare profession.

Great book 5 out of 5 stars.
4 of 5 people found this review helpful.

This is an excellent resource. The illustrations and photographs are wonderful. Carolyn is not afraid to use actual photographs of male and female genetalia....how refreshing! There is nothing worse than medical text using drawings. In real life you will being dealing with the real thing....why not see the real thing in your textbooks! Beyond that...this book is very easy to read and understand. This is a book to keep and use well after you get out of college.
